Cacophonium is an item added by the Botania mod. It can be used to play the sounds of any mob in the game, which can be set by right-clicking on the desired mob. Holding right-click continuously plays that mob's sounds.
The Cacophonium can be used to create a Cacophonium Block by shift+right-clicking a Note Block.
Recipe
Gold Ingot can be substituted with 
Alchemical Brass Ingot.
Usage
Cacophonium has no known uses in crafting.
